As part of the integrated service we have two outreach workers who specialise in providing support to people with dementia in the community.  The outreach workers work with older people with dementia who are reluctant to engage with traditional health and social care services in the community.

They will visit people in their own homes and start by providing a befriending service and gradually introduce health and social care services as the person gradually accepts assistance at home.

The outreach workers will provide the service for up to a 12 week period and will assist in transferring to long term support services that are acceptable to the person and that will meet their needs to support them to remain in their own home.
