Reablement is short-term support that takes place in your own home following a change in your health or care needs. This is to help you become independent and have the confidence to do the things you used to do, so you can stay living in your own home.

To promote independence, we may offer you support with personal hygiene, meal preparation and light chores such as washing up and taking out the rubbish. It’s not about doing things for you, it’s about giving you the skills and confidence to get back to doing everyday tasks and activities yourself, in a safe way.

The Reablement service is free and is for adults who live in the borough of Kingston-upon Thames who are likely to benefit from our support.

It is delivered by staff employed by Your Healthcare, the organisation responsible for community healthcare in Kingston, on behalf of Kingston Borough Council. Our team includes different experts who will help you to get your confidence back and do the things that are important to you in your daily life.

The Reablement Team will work with you to regain your strength and develop the confidence to do the things you used to do.

Trained staff will work with you to identify goals and work out how to achieve them. This often includes support from family and/or friends.

It may also include the use of specialist equipment to help you to become independent and make sure you are safe at home. We can also give you details of other services that you might find helpful, or make a referral for you.

People are usually offered up to 28 hours of support over four weeks, or fewer hours if this level of support is not needed. Support will reduce as your skills and confidence return.

Reablement refers to the short term care you receive after experiencing an illness or injury. Its main aim is to allow you to gain or regain confidence, by acquiring the necessary skills which will enable you to live as independently as possible.

It is provided by our specially trained staff who will focus on observing, guiding and supporting you to do things independently.

The service is available from 7.00 am to 20:00pm 365 days a year.
