Your Healthcare provides support, advice and education for people with diabetes living in the Kingston area.

The service is led by a Community Diabetes Specialist Nurse whose main focus is the DESMOND programme, but she may visit patients at home or in a GP surgery (mainly to support GPs and Community or District Nurses).

. Walking Away from Diabetes - NEW PROGRAMME


DESMOND is a nationwide structured group education programme developed by experienced health care professionals in participation with people who have diabetes.

The programme is for those newly diagnosed with Type 2 Diabetes and there is also Foundation Desmond for those with established diabetes.

Patients will receive 6 hours (9.15am – 4.30pm) of structured education in a group setting for up to 10 participants with a family member or friend.

It is delivered by  two accredited healthcare professionals trained to deliver the course and aims to support the person with diabetes, to become the expert. 

The course has been designed to meet NICE and Department of Health/ Diabetes UK quality standards and criteria for structured education.

At DESMOND you can:

• Talk about your concerns
• Get answers to your questions
• Discuss the options for looking after your diabetes
• Be supported in making your own choices about how you deal with your diabetes
• Find out about what support you can expect from your local health service

About Walking Away from Diabetes 

Walking Away from Diabetes a new three-hour education programme targeting people with either impaired glucose intolerance (IGT) or those at risk of diabetes as determined by the Leicester diabetes score developed in collaboration with Diabetes UK. You can find more information on the programme and the referral form in the links at the bottom of this page.There is also a Diabetes Risk Score tool.


Service can be accessed by referral from a Healthcare professional using the designated referral form.
